Are you having or planning to have a virtual visit?

Appointment must be taken at home or in a stationary place with strong internet connection

Appointment cannot be taken in a moving car or any form of transit

If patient is under 18, the patient and parent/legal guardian must be present

Patient should have on appropriate clothing to check range of motion and/or incision site

Log on 5-10 minutes prior to scheduled appointment time to complete visit pre-check questionnaire and confirm information
