Parent/legal guardian’s if patient is under 18
Prior Reports
Printed or emailed
Health
Insurance Card
Completed Visit
Pre-check Questionnaire
Prior images on a Disc(i.e. X-rays, MRIs, CT scans)
We are unable to access images on cell phones or through links.
Please bring all imaging on a disc or arrange direct transfer with our office staff ahead of your visit.
